The Health Record organizes all your secured personal health information collected from claims data, self-reported data, and the Health Risk Assessment (HRA) into one convenient folder. This module is structured into four different sections that allow the user to view claims data, self-report personal health information, print a Portable Health Record and access the Health Risk Assessment questionnaire. The sections are presented as sub-folders, allowing the user to easily move from section-to-section.

Clinical History

View all your personal claims data that have been loaded into
the secured portal. The claims are sorted by date and you can
filter by type of claim including Inpatient, Outpatient, Vision,
Dental, and Pharmacy claims.

allergies, medications, surgeries, family history and any
immunizations you would like to track.
