The Consumer Health Management Center (CHMC) is a secure, web-based personal ‘health home' for individuals and their families. CHMC consolidates fragmented information into a single trusted portal that drives consumer engagement and personal responsibility with an accelerated flow of actionable information.

 

At its core, the CHMC is built on a data warehousing capability that is fully HIPAA compliant and is where the Personal Health Record (PHR) resides. By intelligently integrating the PHR with CHA's Analytic Engine, a full range of structured analytics, messages and decision tools are produced.

 

All actionable intelligence is based upon the individual's personal history and experience and is compared to recognized clinical practice guidelines, prevention & fitness standards and value purchasing benchmarks. The CHMC also offers a range of trusted advisory services and resources, recognizing that consumers need advice and help when they have questions.

All analytics are built upon a robust, pre-populated and automatically updated Personal Health Record. Health Status enables improved self- management of the individual and/or family ‘portfolio' of existing diseases and conditions. Alerts and reminders are immediately brought to the consumer's attention based on key variances from established standards of care.

Tools provided range from analyzing and choosing the best health plan based on medical history, to recommended prescription drug savings options and re-adjudication of disputed claims.
