The Consumer Health Management Center (CHMC) consolidates the consumer's fragmented health record into a
single trusted portal, applies analytics based on standards and resources, and transforms it into actionable
information that enables consumer engagement and responsibility. The web-based CHMC drives highly
personalized consumer-centric information regarding Health Status, Health Promotion and Health Finance; all built upon a robust Personal Health Record that is pre-populated with administrative claims, clinical diagnostics and
financial information.

CHA further supports consumer engagement through on-demand "concierge" and trusted, independent professional advisory services. The CHMC enables patient-physician collaboration through a Physician Portal, and provides aggregate analytics and reporting for the employer through a Plan Sponsor Portal.
